1. OUR SERVICES

2. NO MEDICAL ADVICE & NO PHYSICIAN–PATIENT RELATIONSHIP
Nothing on this website constitutes medical advice, psychiatric diagnosis, treatment, or professional medical guidance.
Accessing or using the Services does not create a physician–patient relationship with Dr. Sarah Quaratella or Lux Mental Health.
A physician–patient relationship is established only after formal intake review, completion of required consent documents, acceptance into care, and completion of a scheduled appointment.
Submitting forms, inquiries, or emails through the website does not constitute medical advice or establish any clinical relationship.
3. EMERGENCY DISCLAIMER
The Services must not be used for emergency or urgent psychiatric needs.
If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency department immediately.
4. LICENSING & TELEMEDICINE LIMITATIONS
Clinical services are provided only in states where Dr. Quaratella is licensed to practice medicine. Dr. Quaratella is currently licensed in Colorado, Rhode Island, New York, and Massachusetts.
Telemedicine availability varies by jurisdiction. You must be physically located in an eligible state at the time services are rendered.
5. CONFIDENTIALITY & WEBSITE SUBMISSIONS
Website submissions may not be HIPAA-secure. Do not transmit sensitive health information through website forms, contact fields, or email.
6. CONSULTING VS. CLINICAL SERVICES
Lux Mental Health provides both clinical psychiatry services and non-clinical services, which may include consulting, speaking engagements, media appearances, and interviews.
Non-clinical consulting engagements do not constitute medical care and do not create a physician–patient relationship.
7. INTELLECTUAL PROPERTY RIGHTS
All content on the Services is owned or licensed by Lux Mental Health and is protected by applicable intellectual property laws. Unauthorized use of any content is strictly prohibited.
Trademarks associated with Lux Mental Health are the exclusive property of their owner. Unauthorized use of any marks, branding, or proprietary names is strictly prohibited.
8. USER REPRESENTATIONS
By using the Services, you represent that you are legally able to agree to these Terms, you are not a minor in your jurisdiction, you will not use the Services for unlawful purposes, and you will not use bots, scripts, or automated means to access the Services.
9. PROHIBITED ACTIVITIES
You agree not to attempt to gain unauthorized access to the Services, upload malicious software, use content for unauthorized commercial purposes, or interfere with or disrupt site functionality.

16. LIMITATION OF LIABILITY
Ivy Psychiatry PLLC shall not be liable for any indirect, incidental, consequential, special, or punitive damages arising from or related to your use of the Services.
17. INDEMNIFICATION
You agree to indemnify and hold harmless Ivy Psychiatry PLLC from any claims, liabilities, damages, or expenses arising from your use of the Services or violation of these Terms.
18. USER DATA
We may store usage data related to performance of the Services. We are not liable for loss or corruption of such data.
19. ELECTRONIC COMMUNICATIONS & SIGNATURES
Electronic communications, transactions, and signatures satisfy any legal requirement that such communications be in writing.
20. MISCELLANEOUS
These Terms constitute the entire agreement between you and Lux Mental Health regarding use of the Services. If any provision is found unenforceable, the remaining provisions shall remain in full force and effect.
